Services

Our main focus is based on the promotion of healthy relationships, choices, independence, and the wellbeing of the individuals we care for, as well as the protection of their rights.

Personal Care Services (children under 21 years old)

This service is for individuals under the age of 21, with physical or/and developmental disabilities, who are enrolled in Children Medical Services, Wellcare / Staywell and Florida Medicaid (Full Medicaid). This service provides medically necessary assistance and training, in the home, with activities of daily living (ADL): eating, bathing, dressing, personal hygiene, and preparation of meals; and age appropriate instrumental activities of daily living (IADL) to enable recipients to accomplish tasks they would normally be able to do for themselves if they did not have a medical condition or disability.

Respite Care Services (children under 21 years old)

This a service that provides supportive care and supervision to recipients under the age of 21, with physical or/and developmental disabilities, when the primary caregiver is unable to perform the duties of a caregiver. This service is generally used due to a brief planned or emergency absence, or when the primary caregiver is available, but temporarily physically unable to care for or supervise the recipient for a brief period of time. This service will be provided to recipients enrolled in Medicaid Waiver (APD)

Personal Support Services (over 21 years old)

Personal Supports Services provide assistance and training to the recipient, (age 21 or older, with physical or/and developmental disabilities) in activities of daily living, such as in the areas of eating, bathing, dressing, personal hygiene, and preparation of meals. When specified in the support plan, this service can also include heavy household chores to make the home safer, such as washing floors, windows and walls; tacking down loose rugs and tiles; or moving heavy items or furniture. This service can provide access to the community-based activities that cannot be provided by natural or unpaid community supports and are likely to result in an increase ability to access community resources without paid support. This service is designed to encourage community integration and to assist in maintaining a recipient’s own home and property as a clean, sanitary and safe environment. Community outing will be provided when requested by the individual or guardian depending on Individual’s needs, choice and preference. This service will be provided to recipients enrolled in Medicaid Waiver (APD)

Life Skills Services (Companion – over 21 years old)

Life Skills Development – Level 1 (Companion Services) consist of non-medical care, supervision and socialization activities provided to adults (individuals age 21 or older, with physical or/and developmental disabilities). The service provides access to community-based activities that cannot be provided by natural or other unpaid supports and should be defined as activities most likely to result in increased ability to access community resources without paid support. This service may be scheduled on a regular, long term basis. There are not merely diversional in nature, but are related to a specific outcome or goal(s) of the individual. Activities may be volunteer activities performed by the individual as a pre-work activity or activities that connect an individual to his community. This service will be provided to recipients enrolled in Medicaid Waiver (APD)

Homemaker and Companion (elderly)

Homemaker & Companion Services consists on assisting individuals to do housekeeping, cook, run errands, and provide companionship to the elderly and adults with disabilities. They cannot provide any personal care or nursing according to state law. This service will be provided to recipients enrolled in United Healthcare & Florida Community Care.
